Navajo Preference Employment Act

Overview
POSITION SUMMARY
The Lead Public Health Nurse (PHN) will assist in overseeing the Public Health Nursing services including primary care referrals, home assessments, preventative services as well as community screening and educational outreach activities and immunization clinics. Under the general supervision of the Public Health Nurse Director, incumbent will assess and develop educational activities prioritizing community health needs with an emphasis on prevention and community outreach with strategic program enhancement and development. The incumbent will provide day-to-day guidance and support to the PHN Director as required to meet program and community needs.
Ensure compliance with all accreditation standards and practice for PHN and perform functions such as planning, organizing, directing, and reviewing work, addressing personnel matters, managing a budget, establishing and maintaining logistical systems, and accomplishing assignments as required. Serves as a liaison and a leader with chapters, schools, providers, and the public relations department and maintains a healthy collaborative relationship with private and public entities, state, tribal and federal agencies to promote the program’s goals and mission.
Qualifications
NECESSARY QUALIFICATIONS
Education:
Bachelor of Science in Nursing (BSN) from an accredited program
Certification:
• A licensed professional, must have a valid, current, full and unrestricted license to practice from any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States
• Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association
• Must have a current and valid state driver’s license with no restrictions maintained throughout employment

Experience:
Two years of Public Health Nursing experience and three years of acute care experience in an inpatient setting
Other Skills and Abilities:
A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

MENTAL AND PHYSICAL EFFORT

Physical :
The work requires extensive driving between communities, and ability to drive in all types of roads in adverse weather conditions. Involves sitting, standing, walking within schools, clinics, and homes, bending, twist, maintain balance and reach for ¾ of workday. Must have ability to frequently crouch and occasionally driving, climb and kneel. The incumbent must have the ability to lifting, pushing, and pulling up to 50 pounds occasionally using appropriate body mechanics and proper lifting techniques is required. Sensory requirements are far vision, near vision, color vision, depth perception, seeing fine details, hearing normal speech, hearing overhead pages and telephone use. Must have ability of simple grasping, firm grasping and fine manipulation of both hands as well as use of keyboard for prolonged periods of the workday.
Mental :
The work involves assessing the health of communities, individuals, families, and schools. Must be able to prioritize and use good judgment; coordinate a variety of issues while being frequently interrupted; The incumbent must have the ability to concentrate, handle a high degree of flexibility, work alone, demonstrate a high degree of patient and work in areas that are close and crowed ¾ of workday. Incumbent must be to cope and manage stressful situations, make decisions under high pressure and cope with anger/ fear/ hostility of others in a calm way, handle priorities in stressful situation and adapt to work shift frequently and will be able to manage altercations occasionally.
Environmental :
Incumbent may continually be exposed to infectious diseases, to chemical agents, dust, fumes, gases, extremes in temperature or humidity, hazardous moving equipment as well as frequent exposure to unprotected heights occasionally and loud noises frequently.
Responsibilities
ESSENTIAL FUNCTIONS:
• The Lead PHN supports the coordination and delivery of community-based nursing services, provides clinical guidance and mentorship to PHN staff, assists with complex patient care and case management, monitors program activities and quality measures, and promotes collaboration with internal departments and community partners.
• Plans, organizes, manages and evaluates a comprehensive PHN Program in accordance with organizational policies and procedures. Integrates current evidence-based public health practices into program operations.
• Participate in community health assessment and determine priorities for services. Provides leadership, direction and technical expertise in the planning, development, implementation and evaluation of PHN services by continually reviewing and evaluating programs to meet the changing needs of the community.
• Ensure compliance, and evaluating the PHN compliance requirements including performance improvement, quality, and accreditation, regulatory (e.g. Joint Commission, Centers for Medicare and Medicaid Services, Health Insurance Portability and Accountability Act) and related programs in conjunction with the TCRHCC departments.
• Collaborates with Quality Improvement, Infection Prevention, Mental Health, Patient Education, and other departments to support interdisciplinary approaches to population health, regulatory compliance, and program development. Participates in grant development, implementation, and reporting when applicable.
• Develops and maintains collaborative relationships with Tribal, local, state, and federal agencies to strengthen public health infrastructure and improve health outcomes within the service area. Represents the PHN program in community partnerships, coalitions, and interagency initiatives.
• Provides leadership with regard to epidemiological surveys, surveillance of index case and ensures timely investigations. Coordinates with Epi-Response team in mobilizing resources to respond to communicable disease outbreaks.
• Develops, maintains, and is primarily responsible for an annual department budget to include: Capital, Operations, staffing, and the departmental strategic goals.
• Participates in the planning, organizing, and evaluating comprehensive staff development, orientation, education, and performance improvement programs. Conducts the department’s performance evaluations, develops and implements effective educational programs to teach groups and individuals at their level of understanding. Oversees time and attendance
• Conducts regular staff meetings to communicate program updates, clarify work processes, and address operational challenges. Develop structured orientation plans for new employees and ensures qualified staff serve as preceptors to support successful onboarding.
• . Promotes professional development by facilitating staff participation in continuing education and training opportunities. Supports staff in obtaining and maintaining relevant certifications and competencies aligned with national public health standards.
• Ensures accurate and timely documentation within the electronic health record system to support patient care, program monitoring, quality reporting, and regulatory compliance.
• Performs other duties as assigned to support departmental operations, public health initiatives, and organizational goals.
