Navajo Preference Employment Act
In accordance with Navajo Nation and federal law, TCRHCC has implemented an Affirmative Action Plan pursuant to the Navajo Preference in Employment Act. Overview
POSITION SUMMARY
This position will provide services to individuals, families, communities, and schools. The incumbent will work under the supervision of a Public Health Nurse (PHN). This position is responsible for collection and reporting of vital signs, patient charting, community outreach, school health services, home visits and patient education and training under the supervision of registered nurses. Provides unscheduled services at the convenience of patients and families, as required to meet program and community needs. Maintains established departmental policies and procedures, objectives, safety, environmental and infection control standards.
Qualifications
NECESSARY QUALIFICATIONS
Education:
High School diploma or GED
Certification/Licensure:
• Must have and maintain a current, active, and unrestricted Certification, License, or registration as a Nursing Assistant from the State of Arizona
• Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association
• Must have a current and valid state driver’s license with no restrictions maintained throughout employment.

Experience:
One (1) year of Certified Nursing Assistant (CNA) or other direct patient care experience

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

MENTAL AND PHYSICAL EFFORT

Physical:
Work requires extensive driving between communities, and ability to drive in all types of roads in adverse weather conditions. Lifting and carrying is necessary to assist disabled patients in ambulation; moderate physical effort is required in handling bulk supplies and equipment weighing up to 50 pounds in homes, schools, and community. Using appropriate body mechanics and proper lifting techniques is required.
Mental:
Must be able to prioritize and use good judgment; coordinate a variety of issues while being frequently interrupted; and be able to cope and manage stressful situations such as chronic or terminal illnesses, child or elderly abuse, and social issues or concerns among patients and families. Must be able to work with a positive team-oriented attitude with other staff; collaborate with multiple departments and personnel
Must be willing to assist other Community Health departments when able and as needed
Environmental:
Incumbent may be exposed to infectious diseases, chemical agents, dust, fumes, gases, extremes in temperature or humidity, hazardous moving equipment occasionally.
Responsibilities
ESSENTIAL FUNCTIONS:
• Assists with direct patient care and related task. Responsible for obtaining and recording patient information as assigned by PHN (i.e., vital signs, blood sugar, pain assessment, medication information, etc.). Responsible for assisting nursing staff to create, maintain and provide a safe environment.
• Educates patients on specified medical issues within established parameters. Shares the responsibility for reporting any pertinent information or changes in a patient’s conditions or reactions to the PHN. If necessary, a referral may be advised under the supervision and direction of the PHN.
• Provides Navajo interpretation for PHN and medical providers in answering patient questions, gathering patient’s medical history, patient diagnosis, medications, address patient concerns or questions and provide patient education and follow up information. Navajo interpretation can be provided to patients in the home setting, clinic setting, community setting, schools or meetings.
• Participates in Epi-response/emergency preparedness in the community in collaboration with all local, Tribal, State, County, and National Emergency Preparedness Programs.
• Participates in health promotion/disease prevention activities, community health fairs, outreach activities, flu clinics, immunization clinics, school vision and hearing screenings. Assists with community needs assessment survey, identifies community resources, gathers/complies community profile data and prioritizes health needs.
• Responsible for documenting appropriate patient encounter in timely manner within the electronic health record area. Adheres to confidentiality TCRHCC policies, and HIPAA regulations. Documents actions and activities by completing required forms, reports, logs, and patient records.
• Practices safety, environmental, and/or infection control. Fosters a collaborative environment, using a team approach to share ideas and to promote safe, patient centered care.
• Participates in performance improvement activities and data management responsibilities. Participates in program meetings, in-service activities, and committees regarding PHN services. Assists with program goals and objectives based on patient and community needs.
• Carries out routine office like activities and administrative functions such as; assist in making and answering phone calls, electronic health record chart reviews, utilizing computer software to enter immunization information, access patient information, or other computer functions as required by PHN department.
• Keeps vehicle and equipment operating by following operating instructions; daily observations and documentation, troubleshooting breakdowns; maintaining supplies; performing preventive maintenance; calling for repairs.
• Performs general non-direct patient care duties related to PHN services, including cleaning, arranging, and restocking supply cabinets; maintenance of inventory and ordering supplies; cleaning and sorting supplies, instruments and equipment; calling attention to deficiencies in supplies and equipment; Adheres to safety, environmental and/or infection control standards.
• Provides basic department orientation to new staff or other related employee’s under the Public Health Nursing Department.
• Responsible for electronic health records data entry pertinent to patient service role.
• Ensure proper PPE is worn as needed while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
• Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates)
• Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.
• Performs other related duties under the Community Health Division, or as necessary per supervisor.
