Job description

Accountable for effectively fulfilling the goals and objectives of the departments while managing resources effectively and efficiently. Demonstrates knowledge of and utilizes established and evolving biomedical, clinical and social science when providing patient care and/or education of others. Specific responsibilities include staff and department management through sound decision making, fiscal financial planning, maintaining appropriate staffing levels, monitoring competency and quality of services. Responsible for working with the interdisciplinary team to identify patients with complex needs for Case Management and Social Services support. Coordinates and provides patient education, assists with appropriate resource utilization, identifies/advocates for needed resources after discharge and assists with transitions across the care continuum.
