Consent Statements:
1. PHOTO, VIDEO & AUDIO RECORDING
As a volunteer, you may be photographed, filmed, or audio-recorded during volunteer service, community events, and related activities. We may use these materials for organizational purposes including social media posts, newsletters, publications, grant reports, educational, and promotional materials. Your participation in volunteer activities constitutes consent to this use. If you have any objection to being recorded or featured, please notify us in writing before your first shift.
2. CODE OF CONDUCT
As a volunteer, you agree to uphold CBWCHC’s Volunteer Code of Conduct, which includes: maintaining confidentiality of all patient information; treating all patients, staff, fellow volunteers, and community members and partners with dignity and respect; refraining from discrimination of any kind; reporting any concerns or incidents to CBWCHC staff promptly; and acting in alignment with CBWCHC’s mission. Violations of the Code of Conduct may result in removal from the volunteer program.
3. ORIENTATION REQUIREMENTS
Participation as a CBWCHC volunteer requires completion of a mandatory orientation. By proceeding with this screening, you confirm your availability and willingness to complete the required orientation before beginning your volunteer service.
