JOB SUMMARY:
The Quality Manager of the Hospital Authority of Miller County (HAMC) is responsible for the quality of outcomes across HAMC's portfolio of community-based programs and population health initiatives. Reporting directly to the Chief Population Health Officer (CPHO), the Quality Director assists in organizational leadership for population health quality: ensuring that the programs HAMC implements in the community are effective, data-driven, and achieve measurable results that improve the health of the Miller County population and reduce unnecessary hospitalizations. The Quality Manager leads HAMC's population health strategy, oversees the development and performance of community-based service lines, and serves as the primary organizational voice for the connection between quality outcomes and HAMC's mission beyond the hospital walls. This role is enterprise-wide and community-facing in its orientation — it is not a hospital operations role.

GENERAL SKILLS:
• Exceptional communication skills, both written and verbal, with the ability to present complex quality data to diverse audiences including clinical staff, executive leadership, and the Board.
• Additional languages preferred to better serve the diverse Miller County community.
• Advanced proficiency in quality management systems, EHR platforms, data analytics tools (e.g., Tableau, Qlik), and Microsoft Office Suite.
• Strong analytical and critical-thinking skills with the ability to translate data into actionable improvement strategies.
WORKING CONDITIONS:
• Primary work environment is a professional office setting within the hospital facility, with regular presence in clinical areas.
• Expected to manage periods of high stress, competing priorities, and time-sensitive regulatory or accreditation activities.
• Must be available outside standard business hours for community program needs, grant deadlines, partnership engagements, and executive leadership responsibilities.
• Occasional travel for regulatory meetings, quality conferences, and collaborative network engagements.
PHYSICAL REQUIREMENTS & DEMANDS:
• Must be able to sit, stand, and walk for extended periods while conducting rounds and audits throughout the facility.
• Ability to operate standard office equipment including computers, phones, and presentation technology.
• Must be able to communicate effectively in person, by phone, and via video conferencing.
• Occasional lifting of materials up to 20 lbs. (e.g., reports, documentation, survey binders).
• Must have near normal vision and hearing to fulfill all leadership and quality oversight responsibilities.
• Ability to manage demanding schedules including evening and weekend commitments as required.
• Must be able to navigate throughout all clinical and administrative areas of the hospital.
• Occasionally within shift (1–33%): Travel to off-site meetings, community events, or regulatory visits.

JOB SPECIFIC COMPETENCIES:
• Enterprise Quality Oversight: Establishes and maintains a shared framework for quality measurement and continuous improvement across HAMC's enterprise, including community-based programs and service lines within the CPHO domain. Does not direct hospital or nursing home quality programs operationally; instead, maintains dotted-line accountability with those teams to ensure alignment with enterprise quality standards, consistent use of improvement methodologies, and cohesive reporting to executive leadership and the Board.
• Population Health Management: Leads HAMC's population health strategy, including oversight of the AHEAD Model implementation and all population health initiatives at Miller County Medical Center (MCMC) as the HUB. Partners with clinical, operational, and community-based teams to design and execute programs that address the social determinants of health, reduce preventable hospitalizations, and improve health outcomes for the Miller County community. Establishes and monitors population health metrics and ensures alignment with federal value-based care frameworks, CMS rural health priorities, and HAMC's strategic plan. Serves as the organizational lead for population health partnerships with state agencies, federally qualified health centers, and regional health networks. The CPHO uses quality outcome data to continuously evaluate program effectiveness, identify gaps in community health access, and drive interventions that reduce preventable hospitalizations and emergency department utilization across the Miller County service area.
• Community Program Development & Oversight: Oversees the development, launch, and ongoing performance of HAMC's expanding community-based service programs and ensures each program has defined operational standards, staffing models, quality benchmarks, and financial sustainability targets. Partners with the CHIEF POPULATION HEALTH OFFICER (CPHO) and operational leadership to evaluate program readiness, monitor performance against goals, and make recommendations for program continuation, expansion, or restructuring. Provides executive leadership and accountability for program administrators and service line leaders within the population health domain, ensuring new programs are integrated into HAMC's broader quality and strategic framework rather than operating in isolation.
• Data Analytics & Outcomes Reporting: Develops and maintains quality dashboards, scorecards, and performance metrics; interprets clinical and operational data to identify trends, risks, and improvement opportunities, with particular emphasis on population health outcomes, program performance metrics, preventable utilization trends, and social determinants of health data relevant to the Miller County community. Presents enterprise quality findings — inclusive of dotted-line program data from hospital and nursing home quality teams — that will be ultimately reported to the CEO, Board, and executive leadership team.
• Workforce & Program Capability Development: Ensures that leaders and staff within the CPHO's directly managed programs have the training, tools, and quality improvement skills necessary to deliver effective, outcomes-driven community health services. Partners with the HR Director and Staff Development function to integrate population health competencies into enterprise-wide professional development strategies.
• Program Quality Standards: Develops and maintains quality standards, outcome benchmarks, and performance expectations for all programs within the CPHO portfolio. Ensures each program operates with clearly defined quality measures, regular performance review cycles, and documented improvement plans where gaps exist.
• Stakeholder Engagement : Engages patients, families, staff, and community partners in quality improvement initiatives; actively communicates quality outcomes and improvement goals throughout the organization.
• Strategic Planning: Partners with the CPHO and executive leadership team to integrate population health quality objectives and community program performance outcomes into the organization's strategic plan and annual operating goals.
• Business Development & Strategic Growth: Identifies and pursues opportunities to expand HAMC's population health portfolio and community-based service lines, including new program development, service partnerships, and grant-funded initiatives. Serves as an executive champion for growth opportunities aligned with community health needs and HAMC's strategic plan, partnering with operational and financial leadership to assess feasibility, define program models, and advance proposals through the appropriate approval process. Does not have direct management authority over business development staff but provides strategic direction and clinical/programmatic expertise to support growth efforts.
• Community Visibility & Program Outreach: Partners with marketing and communications leadership to develop messaging, campaigns, and community engagement strategies that increase awareness of HAMC's population health programs and expand community participation. Serves as a clinical and programmatic voice in shaping how HAMC's community health mission is communicated externally, including representing the organization at community events, health fairs, and public forums.

Requirements
EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:
• Master's degree in Nursing, Healthcare Administration, Public Health, or a related clinical field required; doctoral degree (DNP, PhD, or MD) preferred.
• Minimum of 1 year of progressive experience in population health management, community health program development, or value-based care strategy, with at least 3 years in a senior leadership role.
• Certification in population health management (CPHM) or equivalent preferred; Certified Professional in Healthcare Quality (CPHQ) accepted with demonstrated population health experience.
• Lean Six Sigma Green Belt or Black Belt certification preferred.
• Experience with population health models, value-based care programs, and CMS rural health initiatives (including AHEAD Model or similar) strongly preferred.
• Demonstrated expertise in quality data analytics, performance dashboards, and outcomes reporting.
• Clinical licensure (RN, MD, or equivalent) preferred but not required.
