Outpatient LPN
Position Summary
The Licensed Practical Nurse will provide clinical support and direct patient care within the scope of licensure to support the delivery of safe, high-quality, patient-centered care in the out-patient practice setting as directed by the provider, Registered Nurse (RN), and Practice Manager. The LPN collaborates with the healthcare team to promote efficient clinical operations, patient access, continuity of care, and positive patient outcomes.
The LPN demonstrates strong clinical and organizational skills in a fast-paced medical office environment, including accurate patient assessment data collection, medical administration, point of care testing, procedural assistance, patient education reinforcement, and clinical documentation. The LPN utilizes sound judgement to recognize changes in patient condition and communicates effectively with the RN and/or provider to ensure timely intervention and coordinated care.
Primary Position Responsibilities
1. Patient Care:
a. Delivers direct patient care within the scope of New Jersey LPN licensure and in accordance with HMG policies, protocols and provider direction.
b. Manages time effectively, prioritizes responsibilities, and supports efficient workflow in a fast-paced clinical environment.
c. Collects, reports, and documents patient assessment data, including vital signs, medical history, allergies, medication reconciliation, and other clinical information.
d. Performs clinical duties including point of care testing, EKGs, phlebotomy, specimen collection, medication and vaccine administration, wound care, dressing changes and other clinical protocols / procedures as applicable to the practice setting.
e. Reinforces patient education regarding medications, procedures, follow-up care, preventive measures, and treatment plans as directed by the provider or RN.
f. Assists with stocking, supply management, room turnover, and maintaining clinical readiness.
g. In this position the LPN will collaborate closely with the Provider and/or RN to support triage activities, patient concerns and continuity of care and effective communication.
h. Recognizes significant changes in patient condition and reacts appropriately, escalating concerns to the RN and/or provider.
2. Patient Safety:
a. Identifies safety concerns, equipment issues, or workflow barriers and communicates appropriately to leadership.
b. Appropriately adheres to infection prevention, OSHA, and universal safety precautions during patient care, medication administration, specimen handling, and disposal of medical waste.
c. Participates in daily safety huddles, lunch & learns, staff meetings and quality improvement initiatives.
d. Supports High Reliability Organization (HRO) principles including situational awareness, speaking up for safety and participation in continuous improvement initiatives.
e. Identifies safety concerns, equipment issues, or workflow barriers and communicates appropriately to the RN and/or leadership.
3. Documentation:
a. Assures all patients are correctly identified prior to providing and/or documenting care.
b. Documents patient care, treatments, procedures, communications, and clinical information accurately and timely in the electronic health record (EHR).
c. Ensures proper labeling, handling and documentation for patient specimens.
d. Maintains required logs and documentation related to medications, refrigerators, freezers, vaccines and point of care testing in accordance with organizational and regulatory standards and takes appropriate action if outside acceptable range.
e. Maintains patient confidentiality in accordance with HIPPA and organizational policy.
4. Professional Responsibility:
a. Practices within the scope of New Jersey LPN licensure and organizational policies.
b. Maintains current licensure, certifications, and required educational requirements.
c. Maintains responsibility for professional development through active participation in in-services, continuing education programs, and competency validation activities.
d. Communicates professionally and respectfully with all patients, visitors providers, and colleagues while keeping customer maintaining a high level of customer service.
e. Demonstrates accountability, teamwork, adaptability, and a commitment to patient-centered care.
f. Other duties as required
Qualifications
Minimum Education :
Required:
Nursing Diploma
Preferred:
Associate’s Degree in Nursing
Minimum Years of Experience (Amount, Type and Variation) :
Required:
1 year experience in healthcare setting
Preferred:
2 years nursing experience
License, Registry or Certification :
Required:
Current New Jersey LPN License, CPR certified or obtain certification within 6 months
Preferred:
None
Knowledge, Skills and/or Abilities :
Required:
Good communication and interpersonal skills, decision making ability, telephone etiquette, detail and multitasking oriented, ability to handle a high volume of patients
Preferred:
Ability to work collaboratively in a fast-paced team environment
