Oncology/Hematology Ambulatory APP
- Charlotte, NC
Job description
What We Offer
The Advanced Practice Provider (NP/PA) will provide comprehensive clinical care to patients with complex hematologic malignancies in a high-volume outpatient setting, working collaboratively with hematology physicians and the multidisciplinary care team. The APP will manage scheduled and urgent patient needs throughout the continuum of care, including new and established patient visits, treatment clearance and monitoring, symptom and toxicity management, review and interpretation of laboratory and diagnostic findings, medication management, and supportive care.
The APP will care for patients with diagnoses such as leukemia, lymphoma, multiple myeloma, myelodysplastic syndromes, and other hematologic malignancies. The APP will also play an integral role in the care of patients undergoing CAR-T cell therapy, bispecific antibody therapy, autologous stem cell transplantation, and allogeneic stem cell transplantation, including:
• CAR-T cell therapy: Participate in pre-treatment evaluation and readiness, lymphodepletion and treatment coordination, and post-infusion monitoring and follow-up. Assess patients for treatment-related complications such as cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), cytopenias, infections, and other acute or delayed toxicities, escalating care appropriately and collaborating closely with the cellular therapy team.
• Bispecific antibody therapy: Support initiation and ongoing management of patients receiving bispecific therapies, including step-up dosing, treatment monitoring, toxicity assessment, and longitudinal follow-up. Evaluate and manage complications including CRS, neurologic toxicity, cytopenias, infections, and other treatment-related adverse effects in coordination with the physician and multidisciplinary team.
• Autologous stem cell transplant: Participate in the evaluation and management of patients throughout the transplant continuum, including pre-transplant readiness, mobilization and collection coordination, conditioning, post-transplant recovery, and outpatient follow-up. Monitor for complications such as infection, cytopenias, mucositis, gastrointestinal toxicities, nutritional and hydration needs, and other complications associated with high-dose therapy and stem cell transplantation.
• Allogeneic stem cell transplant: Provide longitudinal care for patients before and after allogeneic transplantation, including monitoring for infection, cytopenias, medication-related toxicities, immunosuppression complications, and acute and chronic graft-versus-host disease (GVHD). The APP will support complex medication management, symptom assessment, patient and caregiver education, and coordination of care with the transplant physician and multidisciplinary team.
The APP will work closely with physicians, nursing, pharmacy, infusion, transplant and cellular therapy coordinators, navigation, and other multidisciplinary partners to coordinate complex care, provide patient and caregiver education, and ensure continuity across the patient's treatment journey. The position will also support rapid evaluation of acute concerns and early identification of complications that may require escalation to a higher level of care.
This position plays an important role in improving patient access, expanding provider capacity, supporting continuity between the inpatient and outpatient settings, and delivering safe, timely, high-quality care to an exceptionally complex and medically vulnerable malignant hematology, transplant, and cellular therapy patient population.
