Citizens Memorial Hospital (CMH) is hiring a License Practical Nurse (LPN) to join our Care Transitions Department in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.
Position Details
• Status: Full Time
• Work Shift: Days
• Hours: 8:00a.m-4:30p.m

What Your Day Looks Like: Care Transitions answers and returns patient phone calls for CMH primary care clinics. The department is made up of two teams, schedulers and triage nurses, who work closely together.
As a Licensed Practical Nurse or Registered Nurse, you will be:
• Answering and returning patient calls for triage and nursing advice.
• Relaying information and results between patients and their primary care provider.
• Assisting with facilitating medication refills, authorizations and questions.
• Identifying and connecting patients to organizational resources.

Qualifications:
Education: Completion of an accredited program (Licensed Practical Nurse or Registered Nurse)
Experience: Patient Care
Skills: Organization, able to work in a fast-paced environment, multitasking, patient care, strong written and verbal communication.
Licensure/Certification: Active Missouri License as a Licensed Practical Nurse or Registered Nurse.
Physical Requirements: Ability to work at a computer 90% of the day and stand for the remaining 10%
