What You'll Do
• Connect patients to healthcare and community resources

• Support chronic disease management and healthy lifestyles
• Provide health education and advocacy
• Work alongside providers, nurses, and care coordinators
• Participate in community outreach activities

Qualifications
• High school diploma or GED
• Strong communication and relationship-building skills
• Knowledge of community resources preferred
• Bilingual (English/Spanish) preferred
• CHW certification or willingness to obtain certification preferred
