About Christ Community Health Services
Since 1995, Christ Community Health Services (CCHS) has provided high-quality healthcare to underserved communities through distinctively Christian service. As a faith-based Federally Qualified Health Center (FQHC), we serve approximately 60,000 patients each year across our Memphis health centers, offering primary care, dental, behavioral health, pharmacy, and specialty services.
About the Role
Christ Community Health Services is seeking a compassionate, resourceful Community Health Navigator to help individuals, families, and community groups obtain preventive and follow-up care, understand healthcare recommendations, and access health and social-service resources.
Working collaboratively with healthcare professionals and community agencies, the Community Health Navigator helps patients address barriers, manage chronic conditions, complete recommended follow-up, and transition safely after emergency-room visits or hospital stays. The role also coordinates wellness events in underserved areas and models the mission, vision, and values of CCHS.
Why Join CCHS?
• Help patients overcome practical barriers to healthcare and community resources.
• Build trusting relationships with individuals and families while supporting better long-term health.
• Collaborate with providers, hospitals, social-service organizations, and community partners.
• Support care transitions, chronic-condition management, preventive care, and community wellness outreach.
• Contribute to a culture grounded in compassion, service, excellence, and Christian faith.

Our Core Values and What They Look Like in This Role
CCHS team members are expected to demonstrate our core values every day. For a Community Health Navigator, they mean:
• Intentional Love - Building trusting relationships and treating every patient, family member, colleague, and community partner with compassion, dignity, and respect.
• Devoted Service - Following through on patient needs, referrals, barriers, and care-transition activities with reliability and a servant's heart.
• Humble Excellence - Maintaining accurate documentation, following professional standards, and continually improving navigation support while remaining teachable and Christ-centered.

What You'll Do
• Establish trusting relationships with patients and their families while providing general support and encouragement.
• Assess patient strengths and identify barriers to obtaining preventive and follow-up healthcare.
• Collaborate with patients to develop and implement practical plans for accessing recommended care and social-service resources.
• Coordinate with primary care providers, outpatient services, hospitals, and community agencies to support continuity of care.
• Maintain complete and accurate program, outreach, and patient-navigation documentation.
• Support care management activities for high-risk patients.
• Visit hospitalized patients, as assigned, to help facilitate smooth care transitions following discharge.
• Conduct post-discharge follow-up after emergency-room visits and inpatient admissions.
• Contact patients in person or by telephone to confirm completion of required or recommended actions.
• Accompany targeted patients to scheduled healthcare appointments or referral sites when appropriate.
• Coach patients in managing chronic health conditions and practicing self-care, including helping them understand care plans and instructions.
• Encourage patients to become active, informed, and engaged participants in their healthcare.
• Facilitate communication and patient empowerment during interactions with healthcare and social-service systems.
• Reinforce disease-specific education, support medication monitoring within the role's authorized scope, and refer clinical questions to appropriate licensed professionals.
• Coordinate community wellness events targeted to underserved areas.

Professional and Operational Responsibilities
• Greet and interact with patients, staff, visitors, and community partners in a pleasant and professional manner.
• Follow applicable standards, laws, and regulations established by regulatory and accrediting organizations and state and federal agencies.
• Protect patient confidentiality in accordance with HIPAA and the CCHS Confidentiality Agreement.
• Check mail, email, and voicemail regularly and respond to inquiries promptly and professionally.
• Share responsibility for answering phones, cleaning, stocking, and organizing common work areas.
• Assist with employee orientation and training when needed.
• Work a reasonable schedule that may include assignments at different service locations and during the operating hours of CCHS or partner organizations.
• Perform other duties as assigned.

What You'll Bring
• Bachelor's degree in Health, Social Services, Human Services, or a related field; or a high school diploma or GED with four years of experience in a position requiring regular interaction with customers or patients.
• Clinical or practice-management experience is preferred.
• Strong interpersonal, communication, and mediation skills for collaborating with diverse patients, staff members, providers, and community partners.
• Organizational skills and the ability to complete a large volume of work in a systematic and timely manner.
• Initiative, creativity, and sound problem-solving skills for addressing barriers and improving care-navigation processes.
• Ability to maintain concentration and accuracy while managing multiple steps, recurring variables, follow-up needs, and documentation requirements.
• Ability to exemplify the mission and values of Christ Community Health Services in all circumstances.
• No license or certification is required for this position.

What Success Looks Like
• Patients understand their care plans, follow-up instructions, referrals, and available community resources.
• Barriers to preventive care, follow-up treatment, medication adherence, and social services are identified and addressed appropriately.
• High-risk patients and individuals transitioning from emergency or inpatient care receive timely follow-up and coordinated support.
• Communication among patients, providers, hospitals, and community agencies is clear, respectful, and effective.
• Program data, patient contacts, referrals, and follow-up activities are documented accurately and promptly.
• Community wellness activities are organized effectively and reach underserved populations.
• HIPAA, confidentiality, regulatory, safety, and organizational requirements are followed consistently.
• The Christian mission and values of CCHS are reflected in daily interactions and work practices.

• 401(k) retirement plan.
