About the Role:
The Care Management Specialist plays a critical role in coordinating and optimizing patient care to improve health outcomes and enhance the overall patient experience. This position involves assessing patient needs, developing individualized care plans, and collaborating with healthcare providers, patients, and families to ensure seamless care delivery. The specialist will monitor patient progress, facilitate access to necessary resources, and address barriers to care, thereby reducing hospital readmissions and promoting wellness. By leveraging data, and evidence-based practices, the Care Management Specialist contributes to the continuous improvement of care processes and patient satisfaction. Ultimately, this role is essential in bridging gaps between clinical care and community resources to support holistic patient well-being.
Minimum Qualifications:
• At least 2 years of direct patient facing experience in health care setting assisting in patient management or related clinical coordination role.
• Knowledge of healthcare systems, patient care processes, and community resources.
• Excellent communication and interpersonal skills to effectively engage with diverse patient populations and healthcare teams.
• Proficiency in electronic health records (EHR)

Preferred Qualifications:
• Associate or bachelor degree in health or social service related field.
• Experience working with pediatrics populations.
• Experience working with patients with special needs populations and/or chronic disease.
• Bilingual abilities or experience working with culturally diverse populations.

Responsibilities:
• Coordinate communication and collaboration among multidisciplinary healthcare teams, patients, and families to ensure continuity of care.
• Monitor patient progress and adjust care plans as necessary to address changing health conditions or barriers to care.
• Facilitate access to community resources, support services, and healthcare providers to address social determinants of health.
• Document all care management activities accurately and maintain compliance with regulatory and organizational standards.
• Provide education and support to patients and families to empower self-management and informed decision-making.

Skills:
The Care Management Specialist utilizes strong communication skills to work with various team members in health care setting to coordinate services from internal and external agencies. Their role is essential for engagement of the pediatric patient, their family, or other caregivers to understand and adhere to treatment plan. Organizational skills enable the specialist to manage multiple cases simultaneously while maintaining accurate documentation and compliance. Problem-solving abilities are applied to identify and overcome barriers to care, ensuring patients receive timely and appropriate services. Additionally, proficiency with healthcare technology, including electronic health records and data management tools, supports efficient tracking of patient progress and quality improvement efforts.
