Your future role at a glance
• Location: Hoffman Estates, IL
• Facility: Ascension Alexian Brothers Behavioral Health
• Department: Inpatient Unit
• Schedule: Day shift | PRN

How you’ll make an impact in this role
• Oversee primary needs of the patient, including assessment of the client and family/caregivers, and develop a comprehensive plan of care.
• Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
• Monitor patient care through assessments, evaluations and/or patient records.
• Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

What minimum requirements you’ll need
Licensure / Certification / Registration:
• Counselor credentialed from the Illinois Department of Financial and Professional Regulation preferred.
• Social Worker credentialed from the Illinois Department of Financial and Professional Regulation preferred.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:
• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences:
• Master's Degree in Counseling or Social Work
• Experience in Case Management and Discharge Planning highly preferred
