Overview
Provides safe, cost-effective drug therapy for ambulatory patients of Shands HealthCare; plans, organizes, directs, coordinates and controls Ambulatory Pharmacy program; disseminates knowledge regarding medication use to patients and health care providers; supports the educational mission of the hospital by providing experiential training of students.
Qualifications
Minimum Education and Experience Requirements: Advanced degree (M.S., MBA or Pharm.D) or at least 3 years of Pharmacy Management experience in a hospital / homecare / institutional / community pharmacy setting is required. In addition, a residency certificate or 2 additional years of practice experience in a hospital / homecare / institutional pharmacy setting is required. Alternatively, a PharmD / MBA degree combination may be substituted for the Advanced Degree / Residency requirement. Registration as a Pharmacist in the state of Florida, or eligibility for same with current Pharmacist licensure in another state. If licensed in another state, the individual must become licensed as a Pharmacist in the state of Florida prior to performing any act which would be considered the 'Practice of Pharmacy' by the Florida Board of Pharmacy, and in all cases, within 12 months of initial employment. Motor Vehicle Operator Designation: Employees in this position: Will operate vehicles for an assigned business purpose as a 'non-frequent driver' NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job. Please indicate the appropriate operator designation on the Request for Personnel (RFP) form at the time a RFP is submitted to post the position. Licensure/Certification/Registration: Registered Pharmacist in the state of Florida. Additional Requirement: 'As a condition of employment within the Pharmacy Services Department of Shands Hospital all employees must be accessible by telephone. This requires each employee to provide a valid telephone number to the department Staff Assistant, in order for the department to reach them in the event of hospital need and/or emergency'.
