VNA RN - Per Diem

The Holyoke Visiting Nurses Association, Inc. is looking to hire a Per Diem Home Health Registered Nurse. Candidates with strong assessment skills are encouraged to apply. Holyoke VNA is committed to quality care, our community, and to your career, as well as an excellent work-life balance.
As a Per Diem position, the following applies:
• Limited to no more than 20 hours per week
• Required to work 1 weekend day or one on-call shift per month (minimum)
• Required to work 1 major holiday a year, (at a minimum of 4 hours)
• Availability must be provided a minimum of 2 weeks in advance, preferably one month in advance.
• Failure to work agreed upon days/hours is considered to be an unexcused absence, unless prearranged by an immediate supervisor or Human Resources.
• Productivity expectations for an 8-hour shift are 6 home visits.
• Hospice admissions equal 3 visits
• Home care admissions equal 2 visits
• All clinical documentation is required to be completed within 24 hours of the visit(s)

Qualifications:
• MA RN License
• CPR Certified
• Strong clinical skills
• Strong computer skills
• Excellent time management skills
