Transportation Escort
Position Summary:
Under the supervision of the Transportation Coordinator, the Escort is responsible for escorting participants in door through door transportation to the PACE Center, home, medical appointments, or other destinations as part of PACE transportation services. The Escort also assumes responsibility for communications while the van is in motion and reporting any changes observed in the participant’s condition.
EXPERIENCE AND EDUCATION:
• Bilingual with fluency in Hmong
• High school diploma or equivalent required.
• Minimum 1–2 years of experience working with older adults, preferably in a PACE, adult day health, long-term care, or healthcare setting. If not present, training will be provided.

PRE-EMPLOYMENT REQUIREMENTS:
• Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
• Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
