Street Medicine Program Medical Assistant
Reports to Office Manager
Background: Cahaba Medical Care Foundation is a community health center organization providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, and Chilton counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission accredited organization, committed to increasing integration and coordination of behavioral health and primary care. Purpose:
To assist providers and other clinical staff in delivering efficient, high-quality patient care as part of the Cahaba Street Medicine Program. The Medical Assistant will provide clinical, phlebotomy, administrative, and patient support services in community-based and mobile settings while helping connect patients with ongoing medical care and community resources.
Travel to various community outreach locations and assigned sites is required to complete job responsibilities.
Principal Responsibilities:
• Assist providers and clinical staff with patient care in mobile, community, and outreach settings
• Triage patients and obtain and document vital signs
• Obtain and document patient medical history, medications, allergies, and other pertinent information
• Document all patient care and services in the electronic medical record (EMR)
• Prepare patients for examinations, procedures, and treatments
• Assist providers with:
• Performing examinations, procedures, and minor surgeries
• Ordering standard laboratory tests for chronic disease visits
• Completing quality management activities in the EMR
• Reconciling medications and allergies
• E-prescribing medications after appropriate changes have been made by the provider
• Perform phlebotomy, including blood draws, specimen collection, labeling, processing, and preparation for transport
• Perform waived laboratory testing as ordered by the provider upon satisfactory assessment of competency
• Assist with patient testing and screenings, including other diagnostic services upon satisfactory assessment of competency
• Administer medications and vaccines upon satisfactory assessment of competency
• Assist with wound care and remove sutures or staples from superficial wounds, as well as casts, splints, and other external devices, as directed by the provider and upon satisfactory assessment of competency
• Set up, organize, and break down mobile clinical areas and equipment
• Ensure medical supplies, medications, equipment, and clinical materials are available and properly stored for outreach activities
• Clean and stock clinical areas and mobile units and maintain accurate inventory records
• Respond to patient messages and assist with scheduling appointments, procedures, referrals, imaging, and follow-up care
• Assist patients with navigating appointments and connecting with appropriate community resources and services
• Support patient outreach and engagement efforts to promote continuity of care
• Complete patient paperwork, including prior authorizations, referrals, home health paperwork, medical supply paperwork, and other required documentation
• Work through assigned tasks in the EMR and maintain accurate and timely documentation
• Communicate with physician offices, pharmacies, laboratories, hospitals, shelters, community organizations, patients, and other ancillary healthcare providers as directed by the provider
• Provide administrative support as needed, including answering phones, scanning and filing documents, maintaining patient records, and coordinating patient information
• Maintain patient confidentiality and follow all applicable HIPAA, safety, infection control, and organizational policies
• Demonstrate flexibility and professionalism while working in a variety of community-based environments and with diverse patient populations

Qualifications:
• Satisfactory completion of CMCF Medical Assistant Training Program
• Certified Medical Assistant preferred but not required
• Phlebotomy experience or certification preferred
• Previous experience in a medical office, community health, outreach, or underserved population setting preferred
• Excellent customer service and communication skills
• Strong organizational skills and attention to detail
• Basic computer and electronic medical record experience
• Ability to manage time, multitask, and work effectively in a fast-paced environment
• Ability to work independently and as part of a multidisciplinary team
• Ability to adapt to changing environments and community-based work settings
• Compassionate and respectful approach to working with individuals from diverse backgrounds and circumstances
• Valid driver's license and reliable transportation, as required for assigned travel
