Overview of Position:
The Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient’s health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.
Education and Experience:
• Diploma in nursing with 2-years’ experience
• Current licensure through South Carolina State Board of Nursing
• CPR (Cardiopulmonary Resuscitation) or BLS (Basic Life Support) certification required
• Possess excellent customer service skills and be well organized.
• At least 2 years experienced working in a primary or specialty care medical practice.
• Ability to work in a high-pressure environment.
• Experience in Endocrinology/diabetes care preferred.

• Possess the ability to work with patients, families, clinical, non-clinical staff and providers from a variety of backgrounds and lifestyles while maintaining a non-judgmental attitude.
• Must possess excellent oral and written communication
• Must have the ability to collaborate with others to improve quality and address needs
• Ability to take initiative and handle various task simultaneously
• Knowledge of policies, procedures, regulations, and protocols
• Knowledge of data bases (ex: EMR systems)
• Bilingual in English/Spanish preferred
• Maintain current medical knowledge to include pharmacology (medication names and usage) knowledge required
• At least two years of experience in a physician’s office
• Excellent interpersonal skills
• Detailed oriented
• Ability to multitask
• Compassion for patients
• Ability to administer a variety of treatments and medications as directed
• Capable of adapting to computer software
Supervisory Responsibilities:
• This position has no direct reports
Essential Job Functions:
• Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data
• Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.
• Assist patients and providers in obtaining medicinal prior authorizations when required
• Verifies the patient understands the purpose of the visit.
• Providers preventative Care information matching the patients age, gender, and health status
• Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment
• Knowledge of examination, diagnostic and treatment room procedures
• Ability to interpret, adapt, and apply guidelines and procedures
• Basic Critical Care and Chronic Illness Management
• Required to participate in training opportunities
• Helps promote office efficiency and encounter volumes
• Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients
• Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs
• Willingness to learn about AIDS Service Organizations and Community Health Centers
• Assist in training new nurses to the organization in the standard working procedures of HopeHealth
• All other duties assigned
ADDENDUM- Staff Nurse Endocrinology
Qualifications:
Knowledge/Skills: Maintain current general medical knowledge of Diabetes/endocrinology and diabetic care. Will work as part of the health care team to provide support, health information and disease management tools to patients with diabetes in coordination with health care providers. Must be comfortable with use of technology, including electronic health record and database management. Working as an effective member of a team as well as working independently is required. Verbalizes and demonstrates comprehension of diagnostic tests and labs for Endocrinology, Endocrinology/diabetic medications, BS monitoring technologies, and insulin delivery devices. Provides professional nursing care to patients who are receiving services through the Diabetes and Nutrition Institute.
Essential Job Functions:
• Teaching around diabetes self-management including nutrition and wellness information and insulin injection training.
• Downloading of data from glucometers, CGMs, and insulin pumps.
• Timely completion of prior authorizations.
• Responsible for documenting receipt of Drug Assistance Program medication shipments and sign out of DAP meds.
• Performs retinal scans and documentation of results.
• Act as health center resource for diabetic materials/handouts and community resources.
• Schedule tests/procedures for patients per Endocrinologist DI orders.
• Occasional travel to other sites (Irby, Kingstree) providing Endocrinology care.
Physical Requirements:
Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
