How you’ll make an impact in this role
• Empower Patient Resilience: Investigate and assess individual patient priorities to plan meaningful therapeutic interventions, helping individuals and families cope with complex social, emotional, and economic challenges.
• Synchronize Collaborative Care: Consult regularly with team managers, medical providers, and family members to ensure complete alignment with the patient's comprehensive treatment plan and regulatory guidelines.
• Mobilize Community Resources: Connect clients and families with critical community assistance, social agencies, and welfare resources to remove barriers to recovery and long-term stability.
• Architect Discharge Journeys: Support the coordination of concurrent reviews and comprehensive discharge planning to ensure seamless transitions of care and secure post-discharge needs.

What minimum requirements you’ll need
Licensure / Certification / Registration:
• One or more of the following is required:
• Bachelor Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date, or Master Social Worker credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
• Specialist Social Worker specializing in Clinical Practice credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.

Education:
• Bachelor's degree in Social Work required.

Work Experience:
• 3 years of experience preferred.

Additional Preferences:
• Master’s Degree in Social Work (MSW): Advanced clinical grounding from an accredited program is required to manage complex psychosocial caseloads.
• Active State Social Work Licensure: Valid, current state licensure (e.g., LSW, LMSW, or LCSW) in good standing is mandatory for immediate day-one practice.
