Job Summary:
MedStar Health is looking for a Social Worker. The Social Under clinical supervision provides psychosocial assessment, counseling, care coordination, and support to patients with serious
illness and their families. As an integral member of the interdisciplinary care team, this role helps patients and families navigate complex medical, emotional, and social challenges associated with serious illness. The social worker participates in goals-of-care
discussions, advance care planning, provides caregiver support, and connection to community resources while promoting quality of life and patient-centered care.

Primary Duties:
• Collaborates with interdisciplinary care teams to assess patients’ psychosocial needs and develop individualized care plans for patients with serious illnesses and their families.
• Conducts comprehensive psychosocial assessments and provides counseling, emotional support, and therapeutic interventions related to illness adjustment, stress, grief, loss, and caregiving challenges.
• Facilitates goals-of-care discussions, treatment preferences, and advance care planning while participating in family meetings, care conferences, and interdisciplinary team meetings.
• Coordinates care transitions, referrals, and community resources, including financial assistance, behavioral health services, hospice, and other supportive services, while advocating for equitable access to care.
• Provides patient and family education, documents assessments and interventions in the electronic health record, participates in clinical supervision toward advanced licensure, and provides weekend coverage as assigned.
Qualifications:
• Master’s degree in social work from a school accredited by the Council on Social Work Education. (Required)
• Prior work experience not required (Required)
• 1-2 years or more years of medical social work experience preferred. (Preferred)
• Palliative care or hospice social work experience (Preferred)
• Graduate internships or a related field may be considered.
Licenses/Certifications:
• LGSW - Licensed Graduate Social Worker. Must hold an active LGSW/LMSW license in Maryland or the District of Columbia at the time of hire. Must obtain licensure in the second jurisdiction and submit the application within ninety (90) days of hire
• LMSW - Licensed Master Social Worker - State Licensure Must obtain advanced clinical licensure (LICSW or LCSW-C) in the primary required jurisdiction within one (1) year of completing all required advanced licensure eligibility requirements, including clinical supervision and supervised practice hours.
• LCSW-C - Licensed Certified Social Worker – Clinical (Required)
This position has a hiring range of
USD $61,838.00 - USD $111,259.00 /Yr.
