Responsibilities
The Social Work Associate facilitates the transition of a patient and family from one environment to another by providing them with a comprehensive continuous plan of care.
Schedule
FTE: 1.0
Hours: 8:00am-4:30pm
Days: Monday-Friday
Location: Gainesville, FL
Qualifications
Education
Required: Bachelor's degree in human services field; BSW from accredited institution.
Experience
Required:
• Clinical assessment skills and expertise in care coordination and utilization management.
• Person should display initiative, has the ability to be multi-tasking and be able to take on responsibilities.
• Proven ability to problem-solve and be a team member is essential.
