• POSITION SUMMARY
• The Senior Services Care Coordinator functions on behalf of the skilled nursing facility and the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
• POSITION QUALIFICATIONS
• Minimum Education
• Licensed Practical Nurse or Bachelor of Social Work
• Minimum Experience
• 2 years in nursing or medical social work
• Required Registration/License/Certification
• Active Alabama license by State Board of Social Work or Nursing
• Preferred Education
• BSN or MSW
• Preferred Experience
• 5 years nursing or social work experience.
• Preferred Registration/License/Certification
• N/A
