Locations: Wound Care Center Lake Havasu City and may float to Valley View Wound Care, Fort Mohave.

As a Wound Care RN Coordinator , you will be part of a specialized team dedicated to delivering advanced wound treatment and compassionate patient-centered care.

This role offers the opportunity to work closely with physicians and multidisciplinary care teams to develop individualized treatment plans for patients with chronic and complex wounds.
As a Wound Care RN Coordinator, you'll combine clinical expertise, patient education, and relationship-building skills to help patients achieve healing and improve their overall well-being.

How you’ll contribute
An Wound Care Coordinator RN, under the direction of the Wound Care Center Program Director and Clinical Nurse Manager, the Wound Care Clinical Coordinator Registered Nurse (RN) will complete wound consults and serve as a resource to providers, staff and external contacts on issues related to wound care:
• WC Clinical Coordinator will assume inpatient wound care leadership and care coordination role.
• This will include chart review of wound photographs and measurements for accuracy, bedside wound assessment, treatment plan of care coordination with facility providers, staff and administrative personnel.
• This person will seek clarification of definitions, clinical diagnoses, locate source documents as appropriate and locate missing data as needed while maintaining expertise.
• Oversight and completion of Inpatient Wound Consults will include wound treatment collaboration with provider and plan of care development.
• Work with nursing team to address pressure injury prevention protocols as appropriate. Point of contact for patients and providers when wound related concerns arise.
• Will provide resource assistance for discharge planning, according to current wound care protocols.
• Coordinator will be responsible for provider, bedside nursing and new hire education of wound dressing rationale.
• Identify and resolve issues affecting the delivery of wound care services. Manage wounded patient lists and reports, Negative Pressure Wound Therapy equipment management and data tracking for quality assurance studies and monthly reporting. Participation in Performance Improvement initiatives such as pressure injury prevention.
• Regular IDT meeting attendance to assist in the coordination of discharge planning. Attend additional meetings such as hospital Fall/Harms, Pressure Injury Sub-committee, clinical team orientation and wound education.
• The position may also serve as a RN in the Outpatient setting to include Case Manager to a group of assigned Wound Care Center patients, as assigned and depending on the Wound Care Center setup Provides patient care to Wound Care Center patients; to include the patient intake and discharge processes as assigned and according to the hospital and nursing standards.
• Participating in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of the patient care.
• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments as well as other medical care treatment.
• Monitors, records, and communicates patient condition as appropriate to care team, provider, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• May also serve as Case Manager to a group of patients as assigned. Performs hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics Hyperbaric training.
• Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinic and patient needs.
• May function as a Documentation Assistant (scribe) in accordance with Healogics policy. Performs other duties as required.
• Assist in vein clinic as assigned after Healogics Vein Clinic training completed. Every effort has been made to make this job description as complete as possible.

What we’re looking for
Applicants should have a current Arizona RN license or Compact RN License with Arizona endorsement, and possess an Associate’s Degree in Nursing (ADN) from an accredited nursing school. Additional requirements include:
• Education: Bachelor of Science (BSN) preferred
• Experience: Minimum of 2 years clinical and Wound Care nursing required
• Certification: BCLS and ACLS certification(s) required within 30 days of hire.
