Registered Nurse, Navigator Cognitive Care

Regions Hospital is seeking an RN to work in a Nursing Navigator role, Monday-Friday at both our St. Paul and St. Louis Park clinic locations. The Cognitive Care Nurse Navigator serves as a patient advocate and care coordinator for individuals with cognitive/memory diagnosis. This role provides clinical guidance, emotional support, and logistical assistance throughout the continuum of care; from diagnosis through treatment, including disease modifying therapy. The Nurse Navigator works collaboratively with multidisciplinary teams to ensure timely access to services, reduce barriers to care, and optimize patient outcomes and satisfaction. They will ensure that patients and caregivers experience a safe, efficient, and supportive process across the entire treatment pathway.
Work Schedule:
Regular Clinic hours, Monday through Friday
Required Qualifications:
Education, Experience or Equivalent Combination:
• Graduated from an accredited school of nursing (RN).
• Associate’s degree in nursing
• Two years of nursing experience in neurology

Licensure/ Registration/ Certification:
• Current Registered Nurse license in either the state of Minnesota or Wisconsin upon hire. Must provide proof of application for alternate state license prior to start and obtain the second license within 30 days of hire. Both Minnesota and Wisconsin RN licenses must be maintained.
• Current BLS provider status in accordance with American Heart Association (AHA) guidelines. Online only is not accepted.
