• Department: Home Health Therapeutic Services
• Schedule: PRN

How you’ll make an impact in this role
• Deliver compassionate direct patient care during flexible PRN shifts to ensure continuous support and clinical excellence across our community.
• Manage and document individualized care plans to keep the medical team informed and highly responsive to any changes in a patient's condition.
• Coordinate seamless care across disciplines to ensure timely execution of medical orders and foster a collaborative healing environment.
• Assess patient discharge needs to guarantee safe, smooth and effective transitions to the next phase of care.

What minimum requirements you’ll need
Licensure / Certification / Registration:
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
• Licensed Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:
• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Work Experience:
• Two years of hospital or home health experience is a must.
