Position Summary
The Discharge Lounge RN is responsible for facilitating patient throughput and supporting efficient hospital-wide patient flow by coordinating the transition of discharged patients from inpatient units to the Transition Lounge. Working collaboratively with Case Coordination, unit nursing teams, EMS providers, and ancillary departments, this role ensures safe, timely, and patient-centered transitions of care while promoting operational efficiency and capacity management.
Essential Duties and Responsibilities
• Coordinate the transfer of discharge-ready patients from inpatient units to the Transition Lounge via wheelchair or stretcher.
• Collaborate with unit nurses, Case Coordination, and support services to identify and transition appropriate patients to the lounge.
• Serve as a central resource for discharge throughput and patient flow initiatives.
• Provide clinical oversight for patients awaiting transportation or post-discharge services in the Transition Lounge.
• Deliver handoff communication and report to EMS and transportation providers to ensure continuity of care.
• Oversee daily operations and workflow within the Transition Lounge, ensuring effective patient throughput and timely patient departures.
• Lead and support Transition Lounge staff in maintaining a safe, efficient, and patient-focused environment.
• Monitor and address barriers to discharge flow, escalating concerns as appropriate.
• Ensure compliance with organizational policies, patient safety standards, and regulatory requirements.

Qualifications
• Current Registered Nurse (RN) license in the State of Connecticut.
• Experience in patient flow, discharge planning, care coordination, or case management preferred.
• Strong communication, organizational, and leadership skills.
• Demonstrated ability to work collaboratively across multidisciplinary teams in a fast-paced environment.
