POSITION SUMMARY :
The Registered Dietitian (RD) provides specialized nutrition counseling and Medical Nutrition Therapy (MNT) education to patients and families. This includes addressing a variety of conditions such as diabetes, cardiovascular disease, renal disease, malnutrition/weight management, failure to thrive in infants, cancer, gastrointestinal disorders, sports nutrition, and general healthy eating.
The RD will:
• Conduct nutrition assessments and identify patients at nutritional risk.
• Develop, implement, and adjust individualized nutrition plans based on patient needs and established clinical guidelines.
• Assess and evaluate outpatients receiving home tube feedings as required.
• Serve as a resource to nursing and multidisciplinary team members to support coordinated care.
• This role operates in both the School-Based Health and Main Clinic settings and provides care through both in-person and telehealth visits as needed.

Requirements
• Bachelor’s Degree or advanced degree from an accredited institution with a major in Foods and Nutrition.
• Registered Dietitian (RD) by Commission on Dietetic Registration/Academy of Nutrition and Dietetics.
• Active NYS Licensure in Dietetics, or the ability to obtain NYS Licensure.
• Active Driver’s License
• Accredited Dietetic Internship (DI) by the American Dietetic Association (ADA)
• Food Handler’s permit or ServeSafe certification as per state requirements
