Community of Hope is seeking a Program Coordinator, HONEY (Housing Our Newborns, Empowering You) to provide administrative and operational support to the Program Director and Perinatal Case Managers to promote efficient, effective, and consistent case management services for families and individuals participating in the Housing Our Newborns, Empowering You (HONEY) program. The Program Coordinator will coordinate program operations and quality initiatives by maintaining systems, resources, and processes that strengthen service delivery, support staff effectiveness, enhance client engagement, and ensure program compliance and operational excellence. This is a full-time position based out of our Family Health & Birth Center in NE, Washington, DC.

What You'll Do
• Provides administrative and operational support to the Program Director and Perinatal Case Managers to facilitate efficient and consistent program operations.
• Manages and maintains HONEY Program data systems, including Quickbase, HMIS, eCW, CRISP, and Relevant Dashboard and reports. Oversees the development, organization, and maintenance of HONEY’s data infrastructure to ensure information can be collected accurately and efficiently in support of program goals, work plans, evaluation activities, and reporting requirements.
• Monitors program referrals and verifies client eligibility in eClinical Works (eCW) prior to placement on outreach lists. Communicates eligibility concerns or discrepancies to appropriate team members.
• Serves as the liaison to DC Health’s Safe Sleep program for HONEY and helps to coordinate training and distribution of cribettes to eligible clients.
• Coordinates and schedules transportation for eligible clients through Rides for Moms, including submitting ride requests and communicating transportation details as needed.
• Supports client discharges and program exits, including preparing and sending discharge letters and relevant resources, completing required documentation, and ensuring timely closure of cases in eCW.
• Assists with the development, formatting, and maintenance of program documents, presentations, decks, forms, templates, and other program materials.
• Supports recruitment, onboarding, training, and ongoing engagement of CAC members as needed, including assisting with recruitment outreach and facilitating CAC meetings and activities.
• Maintains confidentiality and security of client information and ensures data management practices comply with applicable organizational policies, privacy requirements, and program/funder expectations.
• Promotes the “One Community, One Hope” culture through participating in quarterly culture initiatives.
• Completes other tasks as assigned by supervisor.
Requirements
Must-Haves
• High school diploma or equivalent required.
• Demonstrated cultural competence in communicating with underserved, diverse populations.
• Strong interpersonal, conflict-resolution, and organizational skills required.
• Ability to work flexible hours, including occasional weekends and evenings.
• Ability to work independently and as a team required.
• A valid driver’s license and regular access to a car, as well as proof of auto insurance, required.
• Proof of required vaccinations is required. This includes, but may not be limited to, Flu. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.
Nice-to-Haves
• Bachelor’s degree in social work, public health, public policy, or related field or equivalent experience preferred.
• Experience working with families experiencing homelessness or perinatal populations is preferred.
