Practice at Del Norte Community Health Center – Open Door Community Health Centers
The Primary Care Physician provides comprehensive, patient-centered outpatient care for a panel of patients across the lifespan in a rural Federally Qualified Health Center (FQHC) setting. Practicing to the full extent of their training and scope, the physician diagnoses, treats, and manages acute and chronic medical conditions, emphasizes preventive care, and coordinates specialty services when needed. Working as part of an interdisciplinary care team, the physician plays a critical role in improving access to high-quality healthcare for an underserved rural community, helping patients receive timely, comprehensive care close to home and reducing the need to travel outside the region for essential medical services.

Crescent City Community Health Center, part of Open Door Community Health Centers, is a Federally Qualified Health Center (FQHC) serving California’s North Coast. Since opening its doors in 1971, Open Door has expanded access to care and built lasting connections within the communities it serves.
Located in the coastal community of Crescent City in Del Norte County, this area is defined by its beautiful bay, surrounding redwoods, and strong sense of community. Residents, alongside neighboring Tribal communities, are deeply engaged in strengthening and supporting the region.
While more remote, this site offers providers the opportunity to practice collaborative, relationship-centered care within a team-based model while serving a diverse and underserved population. Providers build trusted, long-term relationships with patients, playing an integral role in improving access to care in a rural setting.
The community is supported by a local hospital system, regional air service, and proximity to southern Oregon, offering connectivity while maintaining a small-town coastal feel.
Education and Experience
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
• Completion of an accredited residency program in a primary care specialty (Family Medicine, Internal Medicine, Pediatrics, or Geriatrics)
• Minimum of 2–5 years of clinical experience preferred, ideally in outpatient or community health settings, including experience providing preventive care and management of acute and chronic conditions across the lifespan
Certificates, Licenses, and Registrations prior to hire date
• Current, active, and unrestricted California medical license
• Board Certification or Board Eligibility in a relevant specialty
• Current Drug Enforcement Administration (DEA) registration
• Current CPR/BLS certification

Compensation: $234,635.20 to $267,624.00
