Job description

Salary Range: $50.00 - $55.00 per hour
• Provides direct care and health teaching to community dwelling adults and adolescents in accordance with agency policies and procedures
• Communicates with Primary Care Nurse or Clinical Manager regarding patient status and needs
• Attends staff meetings, in-services
• Provides case management for patients when mutually agreed upon
• Performs all other duties that Management may determine necessary
• Reports to Clinical Manager

Minimum Education:
• The Per Diem Nurse must be a registered nurse with an active license to practice in the State of Connecticut.

The Per Diem Nurse may have one of the following:
• B.S. in Nursing
• Associates Degree in nursing
• R.N. from approved diploma school of nursing

Minimum Experience:
• One year experience in acute/sub-acute or home care setting

Other Qualifications:
• The Per Diem Nurse must possess a valid driver's license and personal vehicle.
• Active Registered Nurse license in good standing in the State of Connecticut.
• Provide proof of current certification in Cardiopulmonary Resuscitation from American Heart Association or American Red Cross.
• Physical, mental and sensory abilities sufficient to meet performance criteria with or without assistive devices.
