Community Health Network has an excellent opportunity for a board-certified, family medicine trained advanced practice provider to work in our Community Urgent Care Clinic locations. Community Health Network's Urgent Care Clinics provide high quality, convenient urgent and acute care throughout all regions of Indianapolis and the surrounding suburbs.

• Epic Job DutiesAdvanced Practice Provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine.
