Highlights
Community Health Network seeks a family medicine or adult medicine trained NP or PA to join our Behavioral Health Services team in the Community Mental Health Center setting. This provider will provide primary care services within our Community Support Services clinic located on the East side of Indianapolis. Ideal candidates will be independent, compassionate, highly accountable, and have a desire to deliver direct and collaborative patient care. Provider experience working with patients experiencing serious mental illness is strongly preferred.
Licensure & Certification
Educational background consistent with requirements for licensure and certification within the state of Indiana.
