CareOne in partnership with BridgeCare Provider Group is Now Hiring - Nurse Practitioner - Internal Medicine
We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide comprehensive medical care to residents in a skilled nursing facility (SNF). The NP will evaluate and manage acute and chronic medical conditions, develop individualized treatment plans, coordinate care with the interdisciplinary team, and promote quality outcomes for a medically complex, primarily older adult population. Travel will be required.
Salary Range $125,000 to $135,000 annually
Compensation will be based on, but not limited to, experience, qualifications, credentials and any other relevant information
Advanced Practice Nurse will be responsible for, but are not limited to:
• Conduct visits to patients in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team.
• Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education:
Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Professional Experience:
• Must have a Nurse Practitioner's license in the state of operations
• Must have 2+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
• Must have a valid driver's license and a reliable source of transportation
• Must be willing to travel
• Experience in a nursing home, hospital, assisted living or acute rehab space is strongly PREFERRED
• Commitment to ongoing professional development and clinical excellence
