Kedren Community Health Center, Inc. is seeking a Nurse Practitioner to provide behavioral health services to adults enrolled in our AB109 and Adult Full-Service Partnership (FSP) programs.
This is a field-based position serving adults who may experience serious mental health needs, housing instability, substance use concerns, justice-system involvement, and other barriers to care. The Nurse Practitioner will work collaboratively with a multidisciplinary team to provide assessment, medication management, treatment planning, ongoing monitoring, and coordination of care in community-based settings.
Key Responsibilities
• Conduct clinical assessments and develop individualized treatment plans.
• Provide medication management and monitor client response to treatment.
• Diagnose and manage conditions within the Nurse Practitioner’s scope of practice.
• Order and review appropriate laboratory and diagnostic testing.
• Maintain timely, accurate, and complete clinical documentation.
• Coordinate care with physicians, behavioral health providers, and other members of the treatment team.
• Participate in case conferences, treatment planning, and multidisciplinary team meetings.
• Provide client education and support related to treatment and medication management.
• Deliver services in field and community-based settings.
• Support continuity of care by addressing barriers and connecting clients with appropriate services and resources.

Qualifications
• Current California Registered Nurse (RN) license and Nurse Practitioner certification.
• Active DEA registration and prescriptive authority.
• Current CPR certification.
• National Board Certification or currently in process.
• Minimum of one year of relevant clinical experience.
• Behavioral health, community mental health, or serious mental illness experience preferred.
• Comfortable providing services in field and community-based environments.
• Strong communication, clinical documentation, organizational, and teamwork skills.
