Manager, Rehab Case Management
Are you a collaborative healthcare leader who is passionate about improving patient outcomes and supporting exceptional care teams? WakeMed is seeking a Manager, Rehab Case Management to lead and oversee the daily operations of our rehabilitation case management department.
In this role, you will guide a team dedicated to helping patients achieve the best possible outcomes throughout their rehabilitation journey. You will be responsible for ensuring operational excellence, clinical quality, financial performance, and an outstanding patient and family experience.
Working closely with patients, families, physicians, staff, and rehabilitation leadership, you will help shape the future of rehabilitation services by driving strategic initiatives, supporting program development, and ensuring compliance with regulatory and accreditation standards.
What You'll Do
• Lead and manage the clinical, operational, and financial performance of the Rehab Case Management department.
• Foster a culture of excellence, collaboration, and patient-centered care.
• Partner with physicians, interdisciplinary teams, patients, and families to support positive patient outcomes and experiences.
• Contribute to strategic planning and department growth initiatives.
• Support program development and continuous improvement efforts.
• Ensure compliance with regulatory, accreditation, and organizational standards.
• Collaborate with Rehab leadership to integrate services and improve care delivery across the health system.
