And above all, it means exceptional care, simply delivered — and we couldn’t do it without you.Make a DifferenceThe Medication Reconciliation Technician will obtain and document medication histories in the electronic medical record in an accurate and timely manner in compliance with standard procedures, the Joint Commission, and other regulatory standards. Your exceptional skillsInterpret and enter “Prior to Admission” medication history into the EPIC electronic medical recordPerform thorough interviews with patients and family membersConduct telephone interviews with outpatient pharmacies, long-term care facilities, or other locations that may provide medication historyTrack interventions and productivity using department’s processRegularly and professionally communicate with patients, their families, and hospital staff Your exceptional qualificationsHigh School diploma or GED equivalent required.Licensed as a Pharmacy Technician by the Indiana Professional Licensing Agency (IPLA)National Certification through the PTCB or NHA required within two years of hire OrLicensed as a Pharmacy Intern by the Indiana Professional Licensing Agency (IPLA). If licensed as Pharmacy Intern, you do not need to be nationally certified with PTCB or NHA
