What We Offer
The medication management specialist serves as a part of the healthcare team at Novant Health and collaborates with other team members, to provide optimal care. The medication management specialist accepts responsibility, authority, and accountability for management and provision of care in accordance with the current policies and procedures. Key component of job includes being responsible for exercising judgment and professional etiquette when interacting with clients/customers. Further responsibilities include assessing and collecting data from customers to resolve prescription status issues. This position is attractive to individuals who enjoy providing quality customer service, are detail-oriented, works well in a team and results driven environment and will convey professionalism, assurance/confidence with every interaction.
What You'll Do
Patient Care: Virtual/telephonic interactions with patient's and/or caregivers: Interviews patient's to collect a current medication list, reviews patient's medication list for accuracy/adherence/discrepancies, assists with medication affordability and adherence, appropriately documents within electronic health record, and collaborates with supervising pharmacist.
Centralized refill: Authorizes prescription refills by adhering to approved refill protocols, appropriately notifies pharmacist/physician when necessary, calls patients to give status updates, and schedules future visits for patients with provider. Follows applicable policies and procedures related to prescription management. Demonstrates proficiency with equipment and software. Effectively communicates with patients, caregivers, team members, and third parties to ensure optimal medication management.
Medication management: Compile, review and sort all insurance requests for medication management reviews on behalf of the pharmacist to assist with closing care gaps, track and monitor requests to ensure appropriate follow-up, and provide support resolving claim reimbursement issues.
Teamwork and Communication: Identifies opportunities to improve processes. Collaborates with team members to achieve desired outcomes. Maintains environment that supports and respects diversity and inclusion to promote creative problem solving. Demonstrates the courage to practice with a questioning attitude. Trains, mentors, and assists in the orientation of new team members.
Compliance and Organizational Alignment: Supports Novant Health policies, system-wide and department initiatives, regulatory and accreditation standards, and quality/process improvement efforts. Successfully completes, in required timeframes, annual competency requirements to support training and education, accreditation, and licensure.
Quality: Supports department and system-wide requirements for compliance with Novant Health policies, regulatory and
accreditation-regulated programs, and quality/performance improvement efforts within the organization.

Round on patients to create meaningful connections and keep patients informed of visit details (delays/wait times). What We're Looking For
• Education: High School Diploma or GED, required.
• Experience: Minimum of five years experience in pharmacy, required.
• Licensure/Certification: PTCB Certification and NCBOP License, required.
• Additional Skills (required): Considerable knowledge of medication names (both trade and generic) and their doses, routes of administration and adverse effects. Demonstrates patient interview skills and strong written and verbal communication skills with other healthcare professionals. Ability to follow verbal and written instructions. Basic technology skills. Other duties asoutlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed.
