JOB DESCRIPTION
POSITION: Patient Care Coordinator
DEPARTMENT: Platte Care Center

Demonstrates the knowledge and skills necessary to provide care for the population of patients in the department.

JOB SUMMARY: Serves as Assistant Director of Nursing, Staff/Resident education, Resident Safety Committee, Resident Restraint Committee. Coordinates the MDS 3.0 process, completes the MDS 3.0 and transmission. Responsible for Admissions.
ESSENTIAL FUNCTIONS:
• Assists Director of Nursing with daily management of the Department.

2. Assists DON with the supervision of CNA’s and Nurses.
• Facilitates Care Committee and Patient Safety/Risk Management Committee meetings as assigned.
• Provides input specific to employee evaluations.
• Cover Admissions for new residents to the facility.

• Provides daily resident rounds on all residents in the facility.
• Completes monthly MDS 3.0 schedule from state printout and updates as necessary (with Ward Clerk).
• Conduct restraint review and ensure compliance monthly.
• Posts schedules for nursing quarterly charting (i.e. pain, oral, nurse observation, etc.)
• Coordinates completion of all CAA’s, Outcomes and AIMS assessment.
• Prepares and updates resident/patients care plans.
• Attends Resident care conferences.
• Completes MDS 3.0 for ward clerks to transmit on a timely basis.
• Available to residents/patients and/or family members for questions regarding assessments.
• Provides information/training R/T documentation needed for MDS 3.0
• Follows HFCA guidelines for the MDS 3.0 process.
• Ensures immunizations status of residents and coordinates annual influenza vaccine with Infection Prevention nurse.

JOB QUALIFICATIONS:
• Education: Currently licensed as a Registered Nurse in South Dakota.
• Experience: Clinical and supervisory experience in LTC preferred.
• Physical Abilities: Work is medium to heavy demand requiring exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently and/or up to 10 pounds of force consistently to push, pull, lift, carry or otherwise move objects. Approximately 80% of shift is spent standing, and/or walking. Ability to grasp. Majority of work requires manual dexterity. Visual acuity and adequate hearing is needed to perform essential job functions. Hands may come in contact with corrosive cleaners.
• Mental Abilities: Must possess excellent communication and organizational skills. Ability to concentrate under pressure and possess a memory for detail.
• Working Conditions: Working indoors majority of the time. Occasional outside exposure to current weather conditions.
• Hazards and Risks: Exposure to contagious diseases. Uses standard precautions/personal protective equipment as necessary.
• Equipment: Utilization of equipment consists of, but is not limited to: computers, copy machine, fax machine and various electrical appliances.
