About the Role:
The Licensed Practical Nurse (LPN) Part-time Nights position in a hospital-based Skilled Nursing Facility (SNF) plays a critical role in delivering high-quality nursing care to patients during nighttime hours. This role ensures the safety, comfort, and well-being of residents by monitoring their health status, administering medications, and responding promptly to any changes in condition. The LPN collaborates closely with registered nurses, physicians, and other healthcare professionals to implement individualized care plans and maintain accurate clinical documentation. Working primarily during night shifts, the LPN must be vigilant and proactive in managing patient needs while supporting a calm and therapeutic environment. Ultimately, this position contributes significantly to the continuity of care and positive patient outcomes within the SNF setting.
Minimum Qualifications:
• Current and valid Licensed Practical Nurse (LPN) license in the state of employment.
• Completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Experience working in a long-term care or skilled nursing facility setting preferred but not required.
• Ability to work night shifts and adapt to a flexible schedule.

Preferred Qualifications:
• Experience in hospital-based skilled nursing facilities or acute care settings.
• Advanced certifications such as IV therapy or wound care.
• Familiarity with electronic health record (EHR) systems.
• Strong communication and teamwork skills demonstrated in previous healthcare roles.
• Additional training in geriatric nursing or chronic disease management.

Responsibilities:
• Provide direct nursing care to residents, including medication administration, wound care, and monitoring vital signs during night shifts.
• Assess and document changes in patients’ conditions, reporting significant findings to the nursing supervisor or physician promptly.
• Assist with activities of daily living (ADLs) such as hygiene, mobility, and nutrition support as needed during the night.
• Ensure compliance with facility policies, safety protocols, and regulatory requirements to maintain a safe environment for residents and staff.
• Collaborate with interdisciplinary team members to update care plans and participate in shift handoffs to ensure seamless continuity of care.

Skills:
The required skills enable the LPN to perform clinical tasks such as medication administration, patient assessment, and documentation accurately and efficiently during night shifts. Effective communication skills are essential for coordinating with healthcare team members and providing clear reports during shift changes. Critical thinking and observational skills help the LPN identify subtle changes in patient conditions and respond appropriately to emergencies. Preferred skills, such as proficiency with electronic health records and specialized certifications, enhance the nurse’s ability to deliver comprehensive care and improve patient outcomes. Together, these skills ensure the LPN can maintain a safe, supportive, and responsive environment for residents throughout the night.
