Job Summary
The Licensed Practical Nurse II (LPN II) delivers high-quality nursing care, administers medications, assists with treatment plans, monitors patient health, documents care, and collaborates with the healthcare team to promote optimal patient outcomes and recovery.
Minimum Qualifications
• Current and valid Licensed Practical Nurse (LPN) license.
• Completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification or equivalent.
• Demonstrated clinical experience as an LPN, preferably at least one year.
• Strong understanding of nursing principles, patient care standards, and healthcare regulations.

Preferred Qualifications
• Experience in long-term care, rehabilitation, or acute care settings.
• Additional certifications such as IV therapy, wound care, or geriatric nursing.
• Familiarity with electronic health record (EHR) systems and healthcare technology.
• Excellent communication and interpersonal skills.
• Ability to work flexible shifts including weekends and holidays.

Essential Duties & Responsibilities
• Provide direct nursing care including medication administration and treatments.
• Monitor and record patient vital signs, symptoms, and progress.
• Assist with patient hygiene, mobility, and comfort measures.
• Maintain accurate patient records and documentation.
• Collaborate with healthcare team members to implement care plans.
• Educate patients and families regarding health maintenance and post-treatment care.
• Ensure compliance with infection control and safety standards.
• Participate in continuing education and professional development.

Professional Responsibilities
• Demonstrate compassionate and professional patient care.
• Utilize critical thinking and problem-solving skills.
• Maintain compliance with healthcare regulations and facility policies.
• Communicate effectively with patients, families, and interdisciplinary teams.
• Maintain proficiency with EHR documentation and healthcare technology.

Physical Requirements
• Ability to stand, walk, bend, lift, and assist patients throughout the workday.
• Manual dexterity sufficient to perform nursing procedures and medication administration.
• Ability to respond to patient care needs and emergency situations promptly.
