How you'll contribute
A LPN who excels in this role:
Essential Functions
• Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
• Performs plan of care interventions, including specimen collection and wound care. May administer medication.
• Documents patient care given.
• Assists RN with development of plan of care.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families
• Works as an advocate for the physical and emotional well-being of the patient.
• Schedules appointments and procedures as ordered by the provider.
• Obtains insurance re-authorizations as requested for testing and scheduled procedures.
Non-Essential Functions
• Attends clinic meetings.
• Maintains a clean and safe working environment.
• Reports all incidents, hazards, drug interactions, and initiate appropriate action.
• Completes safety instructional programs.
• Assists other team members when time permits.

What we're looking for
Applicants should have a current state LPN/LVN license and possess an Associate’s degree from an accredited nursing school. Additional requirements include:
• Certifications: BCLS within 30 days hire/transfer
• Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One-year previous experience preferred.
• Licenses: Must have a current Utah LPN/LVN license in the state of practice or Compact License with Utah endorsement, and be in good standing with the Board of Health Professions
• Education: Graduate of an approved practical nursing program
