Job Description:
Schedule: Full Time
Shift: Days 7am-7pm with every other Weekend
Location: Covering our Stuart F Meyer Inpatient Unit, Palm Coast

• Follows CDC isolation guidelines, demonstrates appropriate use of PPE, performs hand hygiene according to policy, and enforces safety regulations Implements care per the Plan of Care.
• Provides direct patient care to ensure comfort and meet changing needs.
• Educates patients and families on end-of-life care and issues.
• Prepares patients and families for approaching death.
• Promotes patient comfort through appropriate interventions.
• Provides emotional support to patients and families.
• Administers medications and treatments as prescribed.
• ssesses and monitors patient conditions regularly.
• Communicates effectively with patients, families, and healthcare team members.
• Documents patient care activities in compliance with Medicare regulations.
• Participates in interdisciplinary team meetings and care planning.
• Maintains accurate and timely patient records.
• Other duties as assigned.

Knowledge, Skills, and Abilities:
• Critical thinking [Required]
• End of Life care [Preferred]
• EMR [Preferred]
• Critical care nursing [Required]

Education:
• Technical/Vocational School [Required]

Field of Study:
• Nursing - LPN

Work Experience:
• 1+ years experience in the health care field [Required]
• Hospice experience [Preferred]
• LTC or Home Health experience [Preferred]

Licenses and Certifications:
• Licensed Practical Nurse (LPN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]
• Auto Insurance (AL) [Required]
• Certified Hospice and Palliative Nurse (CHPN) [Preferred]

Pay Range:
$21.17 - $39.38
