Position Summary
The Licensed Practical Nurse (LPN) provides direct patient care and clinical support under the supervision of the Clinical Supervisor and in collaboration with providers and the healthcare team. The LPN assists with patient assessments, treatments, medication administration, diagnostic testing, patient education, and care coordination to support quality healthcare delivery. Responsibilities and Duties
• Assist with patient assessments, screenings, and collection of clinical information.
• Administer medications, immunizations, and treatments as ordered by providers and within scope of practice.
• Perform clinical procedures and point-of-care testing, including but not limited to A1C testing, EKGs, urinalysis, and other diagnostic services as assigned.
• Prepare patients for examinations, procedures, and treatments.
• Provide patient education regarding medications, testing, treatments, and follow-up care.
• Accurately document patient information, assessments, interventions, and communications within the Electronic Health Record (EHR).
• Review and communicate test results and provider instructions to patients as directed.
• Assist providers with examinations, procedures, and patient care activities.
• Coordinate patient care activities and facilitate appropriate follow-up appointments and referrals.
• Maintain exam rooms, clinical equipment, and supplies to support efficient patient care.
• Adhere to infection control, safety, confidentiality, and regulatory requirements.
• Foster positive communication and professional relationships with patients, families, providers, staff members, and community partners.
• Participate in quality improvement initiatives, staff education, and professional development activities.
• Perform other duties as assigned.

Requirements
Position Qualifications
Education
• Graduate of an approved Licensed Practical Nursing (LPN) program required.

Experience
• Pediatric and Family Practice experience preferred; new graduates will be considered.
• Experience working in a medical office, primary care, community health, or ambulatory care setting preferred.
• Proficient typing skills and ability to efficiently enter information into the Electronic Health Record (EHR).
• Basic computer skills required, including the ability to navigate software applications, manage electronic documentation, and utilize healthcare technology.

Licensure/Certifications
• Current Licensed Practical Nurse (LPN) license in the State of Maryland or compact state required.
• Current Basic Life Support (BLS) certification required.

Physical and Mental Effort
Physical:
Moderate physical effort is required. Position involves prolonged periods of sitting, standing, walking, bending, reaching, and moving between patient care areas. Requires manual dexterity for administering medications, performing clinical procedures, documenting patient information, and operating medical equipment. Ability to lift, carry, push, and pull up to 25 pounds occasionally. Normal vision and hearing, with or without reasonable accommodation, are required to safely perform patient care responsibilities.
Mental:
Frequent periods of concentration, attention to detail, critical thinking, and clinical judgment are required while providing patient care, administering medications, documenting in the EHR, and coordinating follow-up activities. The position requires the ability to prioritize multiple responsibilities, adapt to changing patient needs, and respond appropriately in a fast-paced healthcare environment. Frequent interaction with patients, families, providers, and staff requires professionalism, empathy, and effective communication skills.
Sensory:
Must be able to effectively communicate in spoken English and possess the ability to read, write, and comprehend English. Requires visual acuity sufficient to assess patient conditions, review medical records, administer medications, perform clinical procedures, and safely operate medical equipment.
Environmental and Working Conditions
Work is performed in a modern, well-equipped healthcare clinic environment. Daily interaction with patients, family members, providers, staff members, and community partners is required. Frequent use of medical equipment, computers, telephones, electronic health records, and other healthcare technology is expected.
Potential exposure to bloodborne pathogens, infectious diseases, bodily fluids, communicable illnesses, sharps, and other healthcare-related hazards may occur. Employees are required to utilize appropriate personal protective equipment (PPE) and adhere to all infection control, OSHA, CDC, and organizational safety standards.
The position may involve exposure to frequent interruptions, varying patient volumes, and a fast-paced clinical environment. Adherence to patient confidentiality, workplace safety, and professional nursing standards is required at all times.
