Facility: Community Hospital of Stokes | Location: Danbury, NC | Position Type: Part-Time | Work Schedule: 7:00 p.m. – 7:00 a.m., Night Shift
What You'll Do
• Provide compassionate, evidence-based nursing care to residents in the Skilled Nursing Facility
• Establish personalized relationships with residents and their chosen support systems
• Collaborate with Registered Nurses and other members of the healthcare team to coordinate and deliver individualized care
• Monitor resident condition and communicate changes or concerns to the appropriate members of the care team
• Support residents throughout their health journey while promoting comfort, safety, dignity, and optimal health
• Provide and document nursing care in accordance with established policies, procedures, and standards of practice
• Maintain accountability for care provided as a member of the RN-led nursing team
What We're Looking For
• Education : High School Diploma or GED, required. Graduate of accredited Practical Nursing Program, required.
• Experience : One year of relevant experience, required.
• Licensure/Certification : Appropriate state LPN license, required.
• Additional Skills (required) : Computer skills, Phone call etiquette, Organizational Skills.
