Licensed practical nurse (LPN) - Medical/Surgical
Novant Health Community Hospital of Stokes | Full-Time Nights (7:00 PM - 7:00 AM) | Danbury, NC
Join our close-knit Medical-Surgical team at Novant Health Community Hospital of Stokes, where you'll have the opportunity to make a meaningful impact while caring for adult and geriatric patients in a supportive community hospital setting. Our 10-bed unit maintains an average daily census of 7-8 patients, allowing nurses to build strong relationships with patients and provide highly individualized care.
This unit cares for a variety of medical-surgical patients, including those with COPD, congestive heart failure (CHF), atrial fibrillation (A-fib), gastrointestinal disorders, and diabetes-related conditions. Working alongside an interdisciplinary team, you'll play a key role in promoting positive patient outcomes and delivering exceptional patient-centered care.
What You'll Do
• Perform comprehensive patient assessments and develop individualized care plans.
• Monitor and manage patients with acute and chronic medical conditions.
• Administer medications, IV therapies, and treatments according to provider orders.
• Assess, document, and communicate changes in patient condition.
• Provide patient and family education related to diagnoses, medications, and discharge planning.
• Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate care.
• Support patients in achieving recovery goals while promoting safety and quality outcomes.
• Accurately document assessments, interventions, and patient progress
• Deliver compassionate, patient-centered care that enhances the overall patient experience.

What We're Looking For
• Education: High School Diploma or GED, required. Graduate of accredited Practical Nursing Program.
• Experience: One year of relevant experience, preferred.
• Licensure/Certification: Appropriate state LPN license, required. NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required. CPI certification within 3 months of hire for Behavioral Health departments, required.
• Additional Skills (required): Computer skills, Phone call etiquette and Organizational Skills. Ability to comprehend policies and procedures and collaborate with an interdisciplinary team. Behavioral Health units' requirements only.
• Ability to intervene in a therapeutic manner for behavior patterns demonstrated by patients with a mental illness, personality disorder, substance abuse or developmental disabilities.
• Knowledge of psychiatric medications, side effects and treatment interventions, knowledge of psychiatric diagnosis, mental health concepts, and evidence based therapeutic interventions and treatments. CPI certification within three months.
• Ability to establish and maintain control of violent patients, defend self and others against violent assaults, ability to react appropriately in psychiatric crises, ability use violent and non-violent restraints per policy and procedure.
