Join Premier Community HealthCare Group as a Lead, Health Coach and help patients build healthier habits while supporting a team dedicated to prevention and chronic disease management. You’ll combine patient education, population health outreach, program development, and team support to help patients make meaningful lifestyle changes. This role is an opportunity to grow a health coaching program that reaches patients across the lifespan and strengthens the health of our community.
We are seeking a Lead, Health Coach who wants to:

What You’ll Do
• Develop engaging, evidence-based curriculum for pediatric and adult healthy living classes
• Facilitate interactive group education sessions both virtually and in person
• Analyze population health data to identify care gaps and support targeted outreach
• Provide front-line support and guidance to Health Coaches
• Serve as a subject matter expert for Health Coach policies, procedures, and program improvements
• Provide individual, telephone, virtual, and group patient education
• Case manage patients participating in preventative care and chronic disease programs, including diabetes
• Complete program reporting and collaborate with PCHG leadership and community partners

Qualifications
• Associate degree in public health, health education, nursing, diabetes peer education, or certification as a Community Health Worker required
• Bachelor’s degree preferred
• At least 1 year of patient education experience required; 3 years preferred
• Two years with Premier Community HealthCare Group strongly preferred
• Current BLS certification preferred
• Ability to obtain and maintain Epic certification, annual compliance training, and Diabetes Education training

Working Conditions & Physical Requirements
• Ability to lift 20 lbs. regularly and 30–50 lbs. occasionally
• Ability to sit for extended periods
• Travel between health centers to provide coverage and support
• Direct exposure to computer screens
• May be exposed to contagious/infectious diseases
