HighlightsCommunity Physician Network seeks an experienced Advanced Practice Provider to serve as lead for our Hospice and Palliative Care APP teams. The Lead APP will work in conjunction with the Medical Director Physician to provide strategic administrative leadership for their clinical sites; promote the advancement of the exceptional patient, provider and employee experience throughout their sites; and provide leadership in the alignment of providers with strategic goals, metrics and priorities. The Lead will assist with recruiting, orientation, scheduling, training, communication, and assessing and safeguarding of APPs within the assigned clinical sites.Job Responsibilities:
• With other leadership partners, helps create and implement the department's vision and strategic plan, helps to translate it into tactical actions to achieve success, and helps drive a performance-based culture.
• Reinforces and promotes group culture among peers and works to advance the cultural changes needed to improve all aspects of the organization.
• In collaboration with the Site Medical Director, promotes, assists and advances quality initiatives; partners with providers to incorporate standards of care and initiatives.
• In collaboration with the Site Medical Director, identifies clinically unacceptable practices and targets them for elimination.
• In collaboration with the Site Medical Director, decreases variation by implementing, monitoring and reporting clinical practice guidelines; identifies best practices; and builds processes/systems to manage them.
• Recruits, selects, coaches, mentors, supports, develops and evaluates APPs working on the team they lead.
• Welcomes, integrates, and helps to develop all new members of the team, including new providers, students, residents, and fellows.
• Addresses and resolves APP behavior and performance problems and applies disciplinary/ corrective actions when needed.
• Escalates provider well-being concerns.
• Encourages and reinforces responsible innovation by others.
• Seeks radical improvements of habitual practices for the sake of quality, cost and service improvements.
• Assists administrative leaders with gaining understanding of provider perspectives.
• Serves as an approachable voice to help balance provider and organizational perspectives; facilitates two-way understanding and helps achieve mutually beneficial outcomes.
• Facilitates departmental meetings.
• Decreases clinical variation by implementing and monitoring clinical practice. guidelines, identifies best practices and builds processes/systems to manage them.
• Ensures safety and compliance, EMR procedures, timeliness and other protocols for the staff.
• Works to create efficient clinical services, implement process improvements, and deliver excellence in patient care.
• Ensures adherence to established quality/safety principles and standards established by the Network.

• Educational background consistent with requirements for licensure and certification within the state of Indiana
