Job description

Job Summary
Plans and coordinates intake and referrals for Hendrick Home Health; ensuring the timely receipt and review of accurate patient information; physician orders; and supporting documentation to facilitate appropriate admissions and quality patient care.
Job Requirements
Minimum Education
• LVN

Minimum Work Experience
• 1 to 3 years of experience as an LVN; preferably with experience in home health.

Required Licenses/Certifications
• CPR (BLS)

Required Skills; Knowledge; and Abilities
• Strong communication skills; verbal and written alike.
• Understanding of physician orders and clinical definitions.
• Knowledge of patient retention; ability to differentiate levels of urgency; and organizational skills.
• Knowledge of regulatory standards regarding physician offices and home health; specifically in compliance with Medicare.
