Working closely with hospital discharge planners, registered nurses, physicians, hospice providers, and families, the LPN Care Coordinator facilitates the transition from hospital to home by coordinating services, communicating care needs, and ensuring necessary resources are in place for a safe and seamless discharge. This is a non-hands-on patient care role focused on coordination and continuity of services.
The LPN Care Coordinator plays a meaningful role in supporting patients and families during the transition home by facilitating discharge planning, coordinating home-based services and equipment, and addressing barriers to care. Through collaboration with the interdisciplinary team, this position helps ensure patients and families have the resources and support needed for a comfortable, coordinated transition home.
The LPN Patient Care Coordinator Will:
• Coordinate transitions from hospital to home for hospice patients, ensuring a safe, timely, and seamless discharge process.
• Collaborate with hospital discharge planners, registered nurses, physicians, hospice providers, and community partners to facilitate continuity of care.
• Coordinate home hospice services, medical equipment, supplies, and other resources needed to support patients upon discharge.
• Communicate with patients and families regarding discharge plans, available services, and next steps in the transition home.
• Identify and help resolve barriers to discharge, including service availability, equipment needs, and coordination of community resources.
• Facilitate the exchange of patient information and discharge documentation among hospital staff, hospice teams, and receiving providers.
• Track transition progress, follow up on outstanding needs, and help ensure required services are arranged before the patient returns home.
• Maintain accurate documentation and support compliance with organizational policies, hospice procedures, and applicable LPN scope-of-practice requirements.
