Location Detail: 765 Fairfield Ave Bridgeport (10411)
Shift Detail: This position serves patients throughout the state and may require travel to any service area within our statewide coverage territory.
Under the supervision of certified homecare leadership, this role supervises certified social work services delivered to patients and families for all Hartford Healthcare at Home (HHCAH) certified regions. The Supervisor collaborates with the region’s leadership on the management of patient care, day-to-day operations and department personnel. This position involves direct care of patients and supervision of all social work team members across the state, including staff development, hiring, supervising and evaluating team members. Responsible for supporting the overall quality of patient care, regulatory and internal standards as it relates to the social work program. Works regularly with interdisciplinary teams.
Provides leadership support for all Regional Certified Social Work departments as needed to meet all operational needs of each region across the state. Promptly addresses and resolves staffing and operational issues as they arise. Serves as liaison between patients or families/caregivers and community agencies as needed. Maintains collaborative relationships with organization personnel to support patient care.
Responsible for Social Work staff supervision including scheduling, managing performance, identifying training needs, completing payroll and providing input on hiring. Supervises Social Workers by reviewing documentation and ride along visits to ensure state and federal compliance. Provides orientation for new employees and support to staff members as needed. Ensures productivity, timely evaluation and utilization are maintained for all social work staff across the state.
Performs field social work visits as needed to support the certified social work teams across the state, approximately 3-5 visits per week. Productivity will be based on the needs of the overall certified social work department. Carries out social evaluations and plans intervention based on evaluation findings. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s). Documents patient care and communication per state and federal
compliance. Participates in developing the total care plan and case conferences as required, including discharge planning.
Ensures adherence to company policies, state and federal regulations and safety standards. Participates in the quality assessment and performance improvement program and inservice training. Responsible for tracking key performance indicators (KPIs), analyzing data, and
preparing reports.
Supports the Regional Executive Directors with planning and executing regional budgets related to certified Social Work staffing and patient coverage.
Qualifications
Minimum Requirements:
Master of Social Work from a school of social work accredited by the Council on Social Work Education
• 3-5 years supervisory experience in Home Health
• 3-5 years clinical Social Work experience in a healthcare setting post master’s degree.
• 3-5 years Clinical Social work experience in Homecare setting preferred
• 3-5 years’ experience using an electronic medical record (EMR) system
• Valid CT Driver’s License
• Active Connecticut Clinical Social Work License (LCSW) required

Preferred Requirements:
Master or Doctoral from a school of social work accredited by the Council on Social Work Education
• 5+ years Social Work experience in a healthcare setting post master’s degree.
• 1-2 years leadership/management experience
• Experience in staff supervision, coaching and mentoring
• Previous Epic and HCHB experience 3-5 years or more preferred
• Knowledge of Lean Principles.
