Heart Failure Coordinator

Job description

Job Summary
This position is responsible for coordinating and managing the care of patients with heart failure, ensuring they receive comprehensive and continuous care. This position will be highly involved in quality improvement initiatives, data collection, and reporting to ensure the highest standards of care are maintained. This position collaborates with leaders, providers, and associates to create, implement, and advance innovative clinical programs and processes.
Qualifications
Education
• Required
• High School Diploma, GED, or equivalent
• Completion of an accredited nursing program (LVN)

Experience:
• Preferred
• Two or more years of clinical core measures/quality/performance improvement

Certification/Licensure:
• Required
• Licensed Vocational Nurse (LVN), with authorization to practice in the State of Texas
